What is the Encore Smiles plan all about and how it can benefit you and your family?

It is important to clarify that the Encore Smiles Plan is not dental insurance. The plan’s standard benefits are specifically designed with you and your family’s well-being in mind and the plan is meant only for those who do not have dental insurance and are seeking ways to receive discounts for dental care. Instead of paying the normal offices fees for “à la carte” services, the plan includes a standard set of services designed to maintain and improve your oral health. The plan also includes dental exams and x-rays, both services play an important role in identifying and addressing potential problems early so that costly procedures are avoided. The plan is designed to maintain your oral health and save over the long term. For the non-standard services, the plan offers a flat discount on services comparable or better than many dental insurance providers.

Additionally, different from typical dental insurance providers, the plan has

  • NO yearly maximums
  • NO deductibles
  • NO claim forms
  • NO pre-authorization requirements
  • NO pre-existing conditions/exclusions, basically
  • NO middleman that may get between you and the care you need!

The plan does not require a membership card, it will be effective as soon as you enroll, and it will be valid for a full calendar year. Best of all, all applicants are guaranteed acceptance into the plan!

What are the standard services included in the Encore Smiles plan?

  • Comprehensive exam per year/enrollee (unlimited x-rays as needed)
  • Two dental cleanings per year/enrollee
  • Two dental exams per year/enrollee
  • One emergency appointment per year/enrollee (x-rays included)
  • Fluoride treatment (13 years or younger)

What are the enrollment fees for the Encore Smiles plan?

  • The annual plan fee is $425 for the primary enrollee.
  • For additional enrollees, the annual plan fee is $400.
  • For children under 13, the fee is $340.

All other dental services that are not part of the plan’s standard benefits will get a 20% discount off normal office fees (written financial arrangements are required before treatment is scheduled).

How do I get started and enrolled in the plan?

We are working on setting up a registration link. Until then, please call our office at (770) 977-0977 and a registration form will be sent to you.

What are the terms and conditions of the plan?

(The use of the term “Plan” below refers to the Encore Smiles Plan)

  • Enrollee(s) must pay the full fee at the time of enrollment to be officially enrolled in the Plan.
  • The Plan does not auto renew. Patients who wish to renew will need to complete the enrollment form along with full payment every year.
  • Plan fees are non-refundable.
  • Plan benefits can only be used at Encore Family Dentistry.
  • Patients cannot use both dental insurance benefits and discounted services from the Plan.
  • Plan benefits cannot be transferred to another person.
  • Additional enrollees must have the same mailing address or be part of the main family file (some exclusions might apply if the enrollee is in college).
  • To qualify for the children’s Plan fee, the child must be less than 13 years old at the time of enrollment.
  • CareCredit can be used to pay for plan fees; however, a convenience fee may apply.
  • The Plan is active for one full calendar year from the date of initial enrollment. If the end date of the Plan falls on the day the office is not open, the Plan expires on the next business day the office is open.
  • Procedures and products not eligible for discounts under the Plan include: Electric toothbrushes, Water Flossers, Bleaching treatments, and in office merchandise.
  • Unused plan benefits do not carry over past the Plan last active day. Please keep in mind there are periods in the year when there is a high demand for appointments. We will make our best efforts to fit you in, but to ensure you are able to make full use of your plan standard benefits, we kindly ask you to plan and make appointments with ample time before your plan expires for that active calendar year!
  • To help keep the costs of the Plan as low as possible, we kindly ask that if you need to cancel an appointment, please do so at least 3 days in advance so that we can try to fit another valued patient.
  • Same day cancellations will incur a broken appointment fee of $50 dollars per individual appointment. Multiple same day cancellations may result in the termination of the patient’s enrollment.
  • Encore Family Dentistry reserves the right to refuse treatment and/or terminate the patient’s participation in this Plan without notice or refund, if the patient’s account becomes delinquent or patient is non-compliant with the terms of the Plan.
  • In the unlikely event of a situation these terms and conditions are not clear, management will make the best efforts to find a solution that works for all parties but ultimately, final decision on dispute resolution is up to management.
  • This Plan may be modified, amended, or cancelled at any time without prior notice and may be subject to other terms and conditions.
